Hmmm... question about 2x09 Croatoan

Just finished Croatoan. Loved the brother moments as always. Also loved the scenery at the end where the Impala looked out over the boys as they talked.
I've a question/observation (or something) about the scene where the demon makes the "phone call" to the YED. He says.
And then the comments about "any more tests." So thinking about AHBLp1 when YED tells Sam about his plans for the special kids and that he's pitting them against each other to find out who will be the leader of his army of demons. And he shows how Sam became a special kid by the "flashback" to his cradle. I assume that means that all the special kids had such a visit and were "christened"(?!!) in such a way.
But surely the YED didn't go around "testing" all the kids like he did Sam in Croatoan.
Wait a minute. I've always assumed the call was made to YED. Perhaps it wasn't? Maybe it was to Lilith? Or some other entity that hasn't yet been introduced. I really don't think YED tested all the special kids. So if not, why would he especially test Sam? Maybe someone/thing else was testing Sam.
Hmm...
And on a different note...in the end scene where Sam is trying to get Dean to open up to him, Sam uses the words, "I'm your brother..." I remember back then I was thinking that Sam and Dean either weren't really blood brothers or that Sam was only his half brother. I was convinced of it. Somehow the way Sam stressed those words, "I'm your brother..." just seemed to make me believe even stronger that they weren't full brothers.
However, I haven't really thought about that in a long time and I don't think I really think it anymore. But when I heard Sam say that line again, I remembered how convinced I was.
Still isn't this the season we learn more about Mary? Who knows?
"It's over. You'll be pleased."So I always wondered about the doctor. She was left behind. Does this mean she was a demon? Not sure it's relevant or that it matters. I'm just curious.
"I don't think any more tests are necessary."
"The Winchester boy. Definitely immune as expected."
"Yes, of course. Nothing left behind."
